GLP-1 / Peptide Dose Log: CSV Schema and Template

A CC0 reference schema and template for recording GLP-1 (semaglutide, tirzepatide, retatrutide) and research-peptide doses in a consistent, portable plain-CSV format, with a documented data dictionary. It is intended both as a starting template for individuals keeping their own injection records and as a column specification that tools can import from or export to. The included rows are synthetic examples. The schema mirrors the fields used by the free GLP-1 and peptide dose tracker and reconstitution calculator at glp1.app, so an export from that tool maps onto these columns one-to-one.
