Newborn's pain under the mother's perception

ABSTRACT BACKGROUND AND OBJECTIVES: Pain can generate important complications for the newborn. The mother, inserted in this context, becomes an important ally during the hospitalization since she stays with the. Thus, the study aimed to identify the mother's perception of pain in her hospitalized child in the Neonatal Intensive Care Unit and to compare the mothers' reports with a range of behavioral and physiological signs. METHODS: This is a qualitative study in a tertiary hospital in the State of Ceará. Fifteen mothers who were with their children hospitalized at the Neonatal Intensive Care Unit participated in the study, where they were placed in front of their children in two different moments: at rest and handling, in order to identify signs of pain. The data were analyzed by approximation of the speeches as proposed by Minayo. RESULTS: The results showed that the mothers did not perceive signs of pain in the newborn at rest. However, when handled, the mothers were able to identify the signs of pain through the characteristics presented in the newborn: facial expression, strong crying and the movement of arms and legs. CONCLUSION: Mothers are able to identify signs of pain in the child during painful procedures, mainly through crying and face changes. Thus, it points out the relevance of using pain evaluation scales to measure the behavioral and physiological signs of the newborn in a Neonatal Intensive Care Unit.
摘要:背景与研究目的:疼痛可对新生儿造成多种严重并发症。在此临床场景中,陪伴住院患儿的母亲在其住院期间可成为重要的协作照护伙伴。鉴于此,本研究旨在明确母亲对入住新生儿重症监护病房(Neonatal Intensive Care Unit,NICU)的住院患儿疼痛的认知,并将母亲的主观报告与一系列行为及生理体征进行对比。 研究方法:本研究为塞阿拉州某三级医院开展的质性研究。共纳入15名陪伴NICU住院患儿的母亲作为研究对象,研究设置静息与操作两个不同场景,让母亲位于患儿身旁以识别疼痛相关体征。数据处理采用Minayo提出的话语近似分析法。 研究结果:结果显示,当新生儿处于静息状态时,母亲未能识别出疼痛体征;但当患儿接受操作时,母亲可通过新生儿的特征表现识别疼痛体征,具体包括面部表情变化、剧烈啼哭以及四肢活动改变。 研究结论:母亲可在疼痛操作过程中识别患儿的疼痛体征,主要通过啼哭与面部表情变化。本研究提示,在新生儿重症监护病房中采用疼痛评估量表对新生儿的行为及生理体征进行量化评估具有重要的临床意义。
