**NOTE** This is a single site analysis, but results for all sites are included in the output attached to this record for ease of review & reuse. General observations are recorded below, and details about specific institutions can be discovered in the raw output. - Stroke and elevated blood pressure diagnoses, across the board, were primarily identified with one code (I63.9 and R03.0, respectively) - CKD patients were generally identified most often by E11.22, which is Type 2 diabetes with diabetic CKD, followed closely by codes that identified CKD more as a standalone diagnosis in the N18 branch - Our primary diagnosis in the cohort of T2DM was identified most often via E11.9 (T2DM without complications), though there were some institutions with high occurrences of E11.65 (T2DM with hyperglycemia). This second code was especially prevalent in the pediatric institutions. - Drug code utilization was more varied and depended on prescribing practices. Typically, codes associated with one drug (insulin OR metformin) stand out as most utilized in an institution, and it was less common (though still seen occasionally) for there to be a mix of insulin and metformin codes as the most highly utilized.
