The Osteoarthritis Initiative (OAI) is a longitudinal cohort study designed to investigate the natural history of knee osteoarthritis across the full disease spectrum. OAI includes both clinical data and biological specimens. Collected data encompass knee symptoms and function (assessed using the Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Knee Injury and Osteoarthritis Outcome Score (KOOS)), hip and other joint symptoms, general quality of life, physical performance, knee examinations, risk factors, health behaviors, psychosocial measures, as well as medications and supplements used by participants. At study recruitment, a total of 4,796 participants aged 45 to 79 years were enrolled in the OAI. Collected biological specimens include blood, urine, DNA, and lymphocytes. Imaging data gathered throughout the study include baseline and annual bilateral knee radiographs, bilateral knee MRI, and imaging of other joints (e.g., hip, pelvic, and hand radiographs; thigh MRI; and full-limb MRI for knee alignment assessment). Data were collected during a baseline telephone screening, follow-up clinic visits at 12, 24, 36, 48, 72 and 96 months, mid-term visits at 18 and 30 months, and via questionnaires administered at 60, 84 and 108 months.
