Directory of agencies licensed in Illinois as Home Health Agencies under the Home Health, Home Services, and Home Nursing Agency Licensing Act (210 ILCS 55) and 77 Ill. Adm. Code 245. A Home Health Agency provides skilled nursing services in a patient's residence, according to a physician-prescribed plan of treatment, in combination with at least one other recognized service: home health aide services, physical therapy, occupational therapy, speech-language pathology, or medical social services. This combination requirement distinguishes Home Health Agencies from Home Nursing Agencies, which provide nursing services only. Each record represents one currently licensed agency and includes agency name, location, primary administrative contact, license number, and license expiration date.
