This meta-analysis aims to compare the safety and effectiveness of bivalirudin versus heparin in patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI). Materials and Methods: PubMed, Embase, Web of Science, and the Cochrane Library were systematically searched for randomized controlled trials (RCTs) and cohort studies comparing bivalirudin with heparin in ACS patients undergoing PCI. Meta-analyses were performed for outcomes including all-cause mortality, cardiovascular mortality, major bleeding, major adverse cardiovascular events (MACEs), net adverse clinical events (NACE), myocardial infarction (MI), stent thrombosis (ST), and stroke. Results: A total of 28 studies (11 RCTs and 17 cohort studies) were included, encompassing 584,249 patients. The experimental group (bivalirudin) consisted of 276,391 patients, and the control group (heparin) comprised 307,858 patients.
