Mechanisms of letermovir breakthrough during compassionate primary and secondary prophylaxis were analyzed in 4 patients. Of 3 absolute resistance cases 2 were associated with treatment interruption or low letermovir concentrations in blood. The 4th was not associated with resistance. NGS genotyping confirmed rapid emergence of resistant mutants, before 3 months.
