This study aims to evaluate the regulatory effects of the traditional Chinese medicine formula Qifuyin (QFY) on motor dysfunction, decreased limb strength, aging phenotypes, and gut microbiota dysbiosis in 3xTg-AD mice, a model of Alzheimer's disease, and to explore the associations between microbial changes and motor improvements. By integrating behavioral assessments, aging scores, and metagenomic analysis, the study confirmed that QFY significantly enhances grip strength and motor coordination, reduces aging scores, and restores gut microbiota homeostasis. These effects include restoring species diversity, increasing the abundance of Muribaculaceae, decreasing specific bacteria such as Alcaligenaceae, and modulating microbial metabolic pathways and carbohydrate-active enzymes (e.g., GH43/GH35). Furthermore, these microbial changes were significantly correlated with improved motor function. This research provides new scientific evidence for intervening in Alzheimer's disease-related physical frailty through the gut-muscle/brain axis and reveals the potential mechanism by which QFY exerts its therapeutic effects through modulation of the gut microbiota.
