<b>Background:</b> Painful pelvic symptoms are the main reason why women with endometriosis who do not respond to medical options seek treatment.<b>Aims:</b> To investigate the difference in painful pelvic symptoms before and after laparoscopic endometriosis surgery in patients who do not respond to medical options.<b>Materials and Methods: </b>Women who underwent laparoscopic endometriosis surgery with an indication of drug-refractory painful pelvic symptoms at a large tertiary referral hospital between January 1, 2018 and September 30, 2020 were retrospectively reviewed. A total of 82 eligible participants were included. Patients were divided into two groups: isolated ovarian endometriosis (group 1) (n=51) and deep infiltrating endometriosis (group 2) (n=31). The groups were then compared in terms of visual analogue scale scores for painful pelvic symptoms and various outcomes. Composite intraoperative and postoperative morbidities were primary outcome measures that combined various surgery-related conditions, including transfusion requirements, to draw conclusions.<b>Results: </b>Our preliminary results suggest that laparoscopic endometriosis surgery provided significant pain relief for all painful pelvic symptoms. The magnitude of improvement in visual analogue scale scores after surgery was similar. Therefore, improvement in symptoms was independent of whether patients had isolated ovarian disease or deep infiltrating endometriosis. Composite intraoperative morbidities were significantly higher in group 2 while composite postoperative morbidities was indifferent (p=0.001 and 0.086, respectively).<b>Conclusions: </b>Laparoscopic endometriosis surgery in expert hands brings significant improvement in all painful pelvic symptoms. Modern endometriosis treatment needs to be individualized and modified by adopting a patient-centered, multimodal and interdisciplinary integrated approach.
