CARE Scholars-- Please click the 'Apply for Access' button in the upper right corner of this page, and complete the forms listed there. AND please make sure to ask Dr. Rajeshuni to add you to the 'CARE Scholars-Hospitalization' study on this data portal! (This will take care of the 'Approval - IRB PHS 40974' form and the 'PI - Stanford RIF' form so that you don't have to do anything for those.)
