Abstract This study examines the paradoxical silence of Iran's mental health elite (psychiatrists, psychologists) during the nation's escalating opioid crisis from 1990-2015. Utilizing a political economy framework, we argue that this silence represents a calculated "trade-off" between professional ethics and economic privilege. Despite possessing relative academic freedom during the Reform Era (1997-2005), these elites prioritized private economic gains over public health responsibilities. Through analysis of income data, publication patterns, and institutional behaviors, we demonstrate how a "silence-for-privilege" exchange mechanism enabled the normalization of one of Iran's most devastating public health crises. This crisis disproportionately affects vulnerable groups like truck drivers, with devastating social consequences. Yet during the critical period of 1990-2015, Iran's mental health elite-mainly psychiatrists and psychologists-remained conspicuously silent despite their professional mandate and economic privilege.
