Prepubic urethral transposition in a male dog with extensive stenosis of the pelvic urethra

ABSTRACT We report the use of a new surgical technique in a male dog affected by extensive stenosis of intrapelvic urethra through a antepubic urethral deviation, as an alternative to prepubic urethrostomy and ablation of the external genitalia. The technique consisted initially of orchiectomy, followed by retroumbilical celiotomy, transverse section of the penis in the pre-scrotal region and transposition of this towards the abdominal cavity by making anastomosis to the prostatic urethra. The dog was evaluated clinically and by urethrography retrograde positive contrast for up to four years after the procedure without any clinical signs, changes in urine stream or stenosis image. It is concluded that the pre-pubic urethral transposition is a viable alternative treatment for this dog affected by extensive stenosis of the membranous urethra.摘要：本研究报道了一项新型外科技术的应用案例：针对一例患有骨盆内尿道广泛性狭窄的公犬，采用耻骨前尿道移位术（antepubic urethral deviation）作为耻骨前尿道造口术（prepubic urethrostomy）与外生殖器切除术的替代治疗方案。该手术的初始步骤为睾丸切除术（orchiectomy），随后实施脐后剖腹术（retroumbilical celiotomy），于阴囊前区域横断阴茎，将断端移位至腹腔并与前列腺尿道行吻合术。术后对该公犬进行了长达四年的临床评估及逆行阳性造影尿道造影（retrograde positive contrast urethrography）检查，未观察到任何临床症状、尿流异常或狭窄影像学表现。综上，耻骨前尿道转位术可作为该膜性尿道（membranous urethra）广泛性狭窄患犬的可行替代治疗方案。
