The cost-effectiveness of chronic disease management plans in stroke

The aim of this PhD project was to examine the cost-effectiveness of government-funded policies for managing chronic diseases in Australia, specifically for survivors of stroke. The study focused on the cost and health benefits of chronic disease management plans used by primary care physicians to provide care for patients with stroke. There was evidence that care with the use of chronic disease management plans was less likely to be cost-effective compared to care without such plans. These findings will inform clinical practice and encourage policy-makers to review existing primary care strategies for stroke management.
