This series contains the death registers of the Lottie Stewart Hospital. The Public Health Act 1893 (No. 39a section 1) required that every death be entered into a register. A medical officer of health, under this Act, could inspect the register books of death within his district at any reasonable time. Additionally, the Act required that each death register must contain the cause of death and the name of the person certifying to such cause, and whether such person is a legally qualified medical practitioner. (1)The information captured in this series changes slightly over time. The earliest volume, dated 1954-1966, captures the patient's name, date of death, time of death, witness to death, Doctor for confirmation on Certificate of death, member of staff undertaker and body removed by. In later years, some of the information captured changed, with Sister present in place of body removed by from December 1960 and second Doctor added in place of member of staff from May 1962.The earliest volume in this series also contains some loose clippings of authorisations to remove body at the back of the volume.The later records identify the date of death, patient by name, length of stay, admission date, MRN [Medical Record Number], if death was less than 24 hours, ward/unit, reason/cause of death, if there was an autopsy court case (only from March 1997 onwards), and doctor who signed the certificate.END NOTES(1) Public Health Act 1893 (Act No. 39a, 1893), s. 1.
