This Modified Oral Health Assessment Tool (M-OHAT) captures structured oral health assessment data for aged care residents. The form includes categorical observations on key oral health indicators such as lips, dentures, oral cleanliness, natural teeth, gums, tongue, saliva, odour, and pain. It also records relevant contextual factors like cognitive impairment and assistance with daily activities. The tool includes recommended staff actions, monitoring needs, dental referrals, and assessor comments to support coordinated care planning and follow-up. This tool was co-developed through an interdisciplinary collaboration, with contributions as follows: Prof. Loc Do led the overall project as the Principal Investigator and contributed to the M-OHAT tool development; Dr Kristiana Ludlow led the development of the M-OHAT tool and facilitated co-design workshops; Dr Nicole Walker co-led workshops and contributed to tool development; Dr Laura Ferris supported workshops and tool development; Drs Diep Ha, Helena Schuch, Claudia Lopez Silva, Nicole Stormon, and Emeritus Prof. Laurence J Walsh contributed to tool development as Chief Investigators; Dr Cheuk Candy Fung, Dr Sadaf Khan, Dr Thu Le, Lyndal Pritchard, Dr Lydia See, and Angelique Zamora contributed to tool development; Profs. Andrew Georgiou, Julie Henry, and Saso Ivanovski, Dr Matthew Nangle, Christopher Sexton and Prof. Haitham Tuffaha contributed as Chief Investigators; Steven McNamara supported tool development as an industry partner; Erin Couper, Tamantha Head, Sarah Ramsey, and Leanne Wood contributed as industry partners.
