The emergence of polymyxin-resistant microorganisms has been considered an increasingly worrying factor because it is a class of antimicrobials used as the last choice in the treatment of infections caused by multidrug-resistant gram-negative bacteria. The presence of specific mutations in genes that express proteins of the two-component system (the main component in the mechanism of polymyxin resistance) is a fact commonly observed in clinical isolates of P. aeruginosa that exhibit high levels of polymyxin resistance. As it is an adaptive resistance mechanism, it is also described that resistant isolates return to the sensitivity phenotype when not continuously exposed to polymyxins. The possibility of reversing the resistance phenotype opens possibilities of study to better understand the mechanisms of adaptive resistance to polymyxin in P. aeruginosa, since the influence of these mutations in resistant and revertant isolates is still something less described. In addition, understanding how these mechanisms work may have future implications for therapeutic options against P. aeruginosa infections, with regard to the effectiveness of treatment and prevention of antibiotic resistance.
