HIV Open Data Project: National Minority SA/HIV Prevention Initiative Cohort 10 - Adult Questionnaire Administration Guide

The National Minority SA/HIV Prevention Initiative Cohort 10 - Adult Questionnaire provides the Center for Substance Abuse Prevention (CSAP) and grantees with a tool that can be used to compare participants across grant sites. This questionnaire should be filled out by program participants and is meant to document basic characteristics about the participants and/or changes in their behavior and attitudes relevant to substance abuse and HIV prevention. The survey is comprised of three parts: (1) Facts about You; (2) Attitudes and Knowledge; and (3) Behavior and Relationships. The first section, Facts about You, collects demographic information relevant to CSAP's target population. The second section, Attitudes and Knowledge, collects information about a respondent's attitudes and knowledge toward drug use, alcohol use, sexual behavior, and HIV/AIDS. The third section, Behavior and Relationships, collects information about a respondent's sexual behavior, drug use, and other risk-related behaviors.
