NRS-4999 | Registers of discharges, removals and deaths [Callan Park Hospital]

This series comprises registers of discharges, removals and deaths relating to patients at Callan Park Hospital. They give consolidated information about patients at the time of departure from or death at Callan Park Hospital.These volumes have standard, printed columns headed: date of death, discharge or removal; date of last admission; number in register of patients; name in full; recovered – m/f [male/female]; relieved – m/f [male/female]; not improved – m/f [male/female]; died – m/f [male/female]; assigned causes of death; age at death – m/f [male/female]; and observations.The columns for either discharge, removal or death are marked for each patient, indicating the reason for departure. The Observations column includes coded information in the form of abbreviations or signatures/dates.The Registers of patients and admissions may also be annotated with information about discharges, transfers to other institutions and deaths. The number in the Register of discharges, removals and deaths should correspond with the patient/admissions number for incoming patients. The register entries are arranged chronologically by date of departure or death.
