We describe 3 cases of CTX-M-15-producing extensively drug-resistant (XDR) Shigella sonnei in Los Angeles, CA within 3 months. We show that the 3 highly genetically related isolates exhibit the same antimicrobial resistome and are distinct from domestic and global S. sonnei strains carrying CTX-M genes. The case series highlights the rapid expansion of XDR Shigella in the US and prompt urgent need for adjustments in detection and treatment.
