The Enduring Imprint of Early Adversity: Post-Traumatic Stress Disorder (PTSD) Following Childhood Maltreatment – A Comprehensive Narrative Review of Etiology, Neurobiology, Diagnosis, and Therapeutic Interventions

Childhood maltreatment induces severe psychiatric sequelae, notably Post-Traumatic Stress Disorder (PTSD), distinguished by unique etiological complexities and profound developmental ramifications demanding specialized therapeutic consideration. This comprehensive narrative review elucidates etiological pathways, neurobiological correlates, diagnostic paradigms, and evaluates interventions for PTSD stemming from such adversity. This review is predicated upon a critical synthesis of extant literature, encompassing peer-reviewed journals, monographs, and authoritative clinical guidelines, with prioritization of recent and seminal scholarly contributions. Key findings unequivocally establish the indelible nexus between diverse childhood maltreatment modalities (physical, sexual, emotional, neglect) and PTSD pathogenesis; emotional maltreatment and neglect are potent predictors of adult-onset psychopathology. Prominent neurobiological sequelae include HPA axis dysregulation and structural/functional alterations in critical neural circuits like the amygdala, hippocampus, and prefrontal cortex. Furthermore, PTSD's multifactorial heritability is illuminated, underscoring genetic susceptibility, gene-environment interactions, and familial predispositions. Diagnostic challenges, particularly in pediatric populations, and DSM-5 paradigmatic shifts are scrutinized. Established modalities (TF-CBT, EMDR) are critically appraised against promising, less validated emergent therapies like creative arts interventions.This synthesis asserts the indispensability of a nuanced, developmentally informed PTSD conceptualization for optimizing clinical management and future research. Critical lacunae include insufficient evidence for some emergent therapies. Longitudinal, culturally sensitive research and contextually adapted treatments are imperative for addressing diverse needs and care barriers. This review offers a sophisticated overview to refine therapeutic strategies and alleviate suffering from early life adversity.
