VPRS 14966 Sperm Donor Files, Alphabetical Order by Surname

These files relate to donors who provided sperm for use in the treatment of infertility.Information on file cover was name of donor and their date of birth. Information attached to file included: name of donor, date of birth, the ancestry of the donor (mother, father, maternal and paternal grandparents); physical characteristics of the donor: colour of hair, eyes, etc.; medical history and family medical history.Pathology reports were also placed on file as were copies of the donor statement and consent forms.
