Perception of innovations in Chronic Diseases/Conditions' care: an evaluative research in Curitiba

Traditional biomedical models of health care, geared predominantly for acute and episodic conditions, have not been responding adequately to Chronic Conditions, which require a different, longitudinal and proactive care. This evaluative research about care for Chronic Conditions sought to identify, in a quantitative approach, the perception of users and health teams about care innovations, with a new care model for Chronic Conditions. The results demonstrate a close association between the implementation of the model and outcome changes, such as shared care and self-care. The way care is dispensed is directly related to the level of satisfaction of health teams and local users of health services.
