Eligibility Criteria and Minimum Integrated Care Plan in a Home Care Service: Experience Report from Campo Limpo, São Paulo

This dataset presents a structured clinical-operational framework developed within a home care service in Campo Limpo, São Paulo, Brazil. It includes two core components: (1) eligibility criteria for patient inclusion in a home care program, and (2) a minimum integrated care plan designed to guide clinical management, care coordination, and service organization.The eligibility criteria were developed to support consistent and transparent decision-making regarding patient admission into home-based care, considering clinical complexity, functional status, social context, and the feasibility of safe and effective care delivery outside the hospital setting.
