This data package is a combination of different health related datasets. These include data on vaccination, causes of death and death rate, diabetes, water consumption. The dataset Assertive Community Treatment (ACT) is an Evidence-Based Practice Model designed to provide treatment, rehabilitation and support services to individuals who are diagnosed with a severe mental illness and whose needs have not been well met by more traditional mental health services. The ACT team provides services directly to an individual that are tailored to meet his or her specific needs. The dataset Austin Adult and Children Vaccinations contains information on the number of Vaccine for Children (VFC) and adult safety net (ASN) eligible clients that are immunized, on a monthly basis, through the Austin Public Health ‘Shots for Tots’ and ‘Big Shots’ programs at the St. John and Far South clinics. The dataset Bloomington Animal Care and Control Adopted Animals shows the animal care and control for adopted animals in the city of Bloomington. Bloomington Animal Care and Control's mission is to address and respond to all animal needs in the community through education, enforcement and support in order to build a community where animals are valued and treated with kindness and respect. The dataset Community Emergency Response Team (CERT) program helps train people to be better prepared to respond to emergency situations in their communities. CERT members can also help with non-emergency projects that help improve the safety of the community.

The dataset Diagnosed Diabetes Prevalence contains the number and percentage of diabetes patients in the US during 2013. The prevalence and incidence of diabetes have increased in the United States in recent decades, no studies have systematically examined long-term, national trends in the prevalence and incidence of diagnosed diabetes. The National Healthcare Safety Network (NHSN) Organisms Lists dataset includes provides the NHSN code for each organism and correspondent SNOMED Concept code with the SNOMED Fully Specified Name. The dataset shows the searchable listing of all NJ (New Jersey) licensed child care centers and inspection reports. In New Jersey, child care centers serving six or more children under the age of 13 must be licensed by DCF (Department of Children and Families). The dataset shows the New Jersey deaths due to underlying causes of deaths eligible to be ranked as leading causes of death based on the National Center for Health Statistics standards. Unintentional injury, stroke, chronic lower respiratory diseases (CLRD), Alzheimer's disease, diabetes, septicemia (sepsis), kidney disease, and influenza/pneumonia round out the remainder of the ten leading causes of death for New Jersey. The dataset on Health Codes is from the New York Department of Health and Mental Hygiene. The New York City Department, through its Office of Radiological Health (ORH), regulates radioactive material for medical, research and academic purposes within the five boroughs of New York City. The dataset shows the searchable listing of all NJ (New Jersey) licensed child care centers and inspection reports. Centers are licensed for a three-year period. A copy of the centers’ license is displayed in a prominent location at the center.

The dataset OMHA Receipts provides information about the appeals and claims received by the Office of Medicare and Hearings by Appeal (OMHA) category for Fiscal Year 2011-2013. OMHA administers the nationwide Administrative Law Judge (ALJ) hearing program for appeals arising from individual claims for Medicare coverage and payment for items and services furnished to beneficiaries (or enrollees) under Medicare Parts A, B, C and D. The dataset Studies On Safe Drugs During Pregnancy contains a list of the current Pregnancy Registries in research in the United States, their status and contact information. Pregnancy exposure registries are observational studies that focus on the outcome of pregnancies of women with medical conditions that require taking medications. The dataset West Nile virus (WNV) is most commonly transmitted to humans by mosquitoes. There are no medications to treat or vaccines to prevent WNV infection. Fortunately, most people infected with WNV will have no symptoms. The dataset eICU Collaborative Research Admissions Summary Statistics contains demographic, diagnosis and hospital level summary statistics for 200,859 patient unit encounters corresponding to 139,367 unique patients admitted between 2014 and 2015. The Philips eICU program is a transformational critical care telehealth program that delivers need-to-know information to caregivers, empowering them to care for the patients. The dataset eICU Collaborative Research Database (eICU-CRD) is a multi-center intensive care unit (ICU) database with high granularity data for admissions monitored by eICU Programs across the United States. The eICU Collaborative Research Database is a subset of a research data repository maintained by eRI. A stratified random sample of patients was used to select patients for inclusion in the public dataset.
