All Individual Ambulatory care sensitive conditions (ACSCs) admission rate

Provides a list of all ACSCs (individual and total) for your selected area and\r\nselected year\r\n\r\nACSCs are those for which hospitalisation is thought to be avoidable with the\r\napplication of public health interventions and early disease management,\r\nusually delivered in ambulatory setting such as primary care. High rates of\r\nhospital admissions for ACSCs may provide indirect evidence of problems with\r\npatient access to primary healthcare, inadequate skills and resources, or\r\ndisconnection with specialist services.\r\n\r\n
