Gastric cancer ranks among the most prevalent malignant tumours globally, with Helicobacter pylori constituting a primary risk factor. Nevertheless, only a minority of infected individuals ultimately develop cancer. Presently, the dynamics of H. pylori infection rates during the pathological progression and its carcinogenic mechanisms remain unclear. This uncertainty regarding the mechanism of action may be attributed to variations in the genetic background of prevalent strains across different environmental populations. Yunnan Province, situated along China's southwestern frontier, features complex topography and multi-ethnic communities, lending it distinct regional representativeness. This study, therefore, systematically analyses H. pylori infection rates, phylogenetic lineages, virulence typing, and antibiotic resistance gene distribution among gastric disease patients across urban and rural areas of Yunnan. By elucidating its epidemiological characteristics across different pathological stages, the research aims to provide a reference framework for early gastric cancer risk assessment and personalised treatment strategies within regional populations.
