Adopt-A-Spot Application Survey form

This form was created by the City of Dallas' Office of Data Analytics and Business Intelligence (DBI) for use by Code Compliance's (CCS) Keep Dallas Beautiful campaign. It enables residents to apply for a location in Dallas to adopt and clean, contributing to the city’s efforts in maintaining clean and well-kept public spaces. Once a resident submits the form, an email notification is automatically sent through Power Automate to both the Keep Dallas Beautiful team ([e-mail]) and the resident’s email. This streamlined process ensures prompt communication and coordination, helping to facilitate community participation and support the ongoing success of the Keep Dallas Beautiful campaign.
