The provided document includes: Demographic Data Form: A structured questionnaire collecting maternal age, weight, height, education level, and employment status. Pregnancy Physical Activity Questionnaire (PPAQ): The cross-culturally adapted version used to assess habitual physical activity across four domains: household/caregiving, occupational, sports/exercise, and transportation.
