Lichen sclerosus is a chronic inflammatory-atrophic dermatosis of the anogenital area characterized by itching, scarring, and risk of squamous cell carcinoma. The diagnosis is confirmed by biopsy: hyperkeratosis, epidermal atrophy, subepidermal cleft, collagen homogenization, and lichenoid infiltrate are typical features. The immunohistochemical profile with p53 positivity, low Ki-67, and negative p16/CK17 helps to verify lichen sclerosus and exclude neoplasia.
