This data package contains the Clinical Laboratory Improvement Amendments (CLIA), mammography facilities certified by the Food and Drug Administration (FDA), radiation-emitting electronic products data, Computed Tomography of head and Magnetic Resonance Imaging of head data.

**Description**
This data package contains the records of all commercially marketed laboratory tests that have been categorized under the Clinical Laboratory Improvement Amendments (CLIA), zip code of all mammography facilities certified by the Food and Drug Administration (FDA) or Certifying State as meeting baseline quality standards, names of radiation-emitting electronic products and devices and associated information developed by the Center for Devices and Radiological Health (CDRH) in support of its mission, all Categories names and codes of Radiation-Emitting Electronic Product Codes Database,  Computed Tomography of chest per 1,000 Medicare enrollees for 2011 and 2012, Lumbar Radiofrequency Ablation data form 2003 to 2012.
