Background: Maternal and neonatal deaths can be reduced by ensuring full access to maternity services. Yet, in Ethiopia, evidence indicates low utilization of these services, posing a major challenge. Thus, more evidence is needed on maternity services' continuity at the national level in general, and particularly in Bench Sheko zone where no previous studies have been conducted. Objective: To determine the magnitude and its determinants of the complete uptake of maternity services in Bench Sheko zone, Ethiopia. Methods: From February 1st to April 30th, 2023, community-based cross-sectional study was conducted, involving a total of 848 postpartum women nested in 21 kebeles. Data was collected using pretested structured questionnaires, entered using EpiData version 3.1, and analyzed by STATA version 14. Determinants were identified using multilevel mixed-effect logistic regression analysis. A P<0.05 considered statistically significant. Result: The study found that 50.2% of women fully utilized maternity services. Variables like, attending higher education (AOR = 4; 95% CI: 1.71 - 9.33), decision-making autonomy (AOR = 7.17; 95% CI: 3.61 - 14.25), media exposure (AOR = 2.03; 95% CI: 1.14 - 3.61), and receiving advice on danger signs (AOR = 2.12; 95% CI: 1.54 - 3.31) were positively associated, whereas, women residing in rural areas (AOR = 0.12; 95% CI: 0.02 - 0.59), initiating antenatal care late (AOR = 0.29; 95% CI: 0.12 - 0.72), and living more than five kilometers away from nearest health facility (AOR = 0.20; 95% CI: 0.08 - 0.51) were negatively associated with complete uptake of maternity services. Conclusion: Complete uptake of maternity services in the study area was low as compared to the national target. Thus, urgent need for targeted interventions and improved accessibility to ensure comprehensive and equitable maternity services. It’s important to prioritize female education and empowerment, particularly in rural areas, to address the underlying determinants. By focusing on these factors, we can maximize the uptake of maternity services, reduce maternal and neonatal mortality rates, and make significant progress towards achieving the Sustainable Development Goal by 2030.
