**Overview** This data package shows the Co-Morbidity Among Chronic Conditions, Inpatient Admission ER Visit Zip Code and Value-Based Insurance Design Model by the Centers for Medicare and Medicaid Services (CMS). **Description** This data package provides an overview of chronic conditions with co-morbidity among Medicare FFS beneficiaries. The dataset contains information on selected chronic health conditions in the Medicaid population at the zip code level and Medicare Advantage Value-Based Insurance Design Model. Model is an opportunity for Medicare Advantage plans to offer supplemental benefits or reduced cost-sharing to enrollees with Centers for Medicare & Medicaid Services (CMS)-specified chronic conditions, focused on the services that are of highest clinical value to them. **Benefits** - Useful for providers for the use in the management of different chronic conditions. **License Information** The use of John Snow Labs datasets is free for personal and research purposes.
