Liver cancer is a serious disease that can start in the liver or spread there from other parts of the body. When cancer spreads to the liver, it is called liver metastasis. One of the newer types of cancer treatment is called immune checkpoint blockade (ICB). ICB is a type of immunotherapy which works by blocking proteins (called checkpoints) that normally keep the immune system from attacking the body's own cells, thus allowing the immune system to recognize and attack cancer cells more effectively. ICB has improved survival in many types of cancer. However, some laboratory studies in animals suggest that cancer may not respond as well to ICB when it has spread to the liver. So far, there is limited evidence from human studies to confirm whether this is true. This research aims compare response to ICB in patients with and without liver metastasis to see if there is a difference. The research will focus on whether patients with liver metastasis live as long as those without when treated with ICB (overall survival), and whether tumors shrink or respond to ICB treatment (radiological response) as well in patients with liver metastasis. This study will be a meta-analysis, which means we will combine results from several clinical trials to get a clearer picture of the overall trends. The included trials have compared ICB with other standard treatments, such as chemotherapy (drugs that target and kill rapidly dividing cells like cancer cells), supportive care, or targeted therapies (drugs that target specific proteins on cells to allow them to kill only cancer cells), but not other immune-based treatments. We hope this research will help doctors understand whether those with liver metastasis have reduced benefit from ICB treatment. If that is the case, it may suggest that different types of treatment (such as surgical removal or radiation (where the cancer is targeted with beams of energy) could improve outcomes for these patients. By providing clearer evidence, this project may guide future treatment decisions and improve care for people with advanced cancers that have spread to the liver.
