This dataset includes data that is publicly accessible and obtained from the Chronic Conditions Data Warehouse: https://www2.ccwdata.org/web/guest/condition-categories-chronic.
The dataset contains 30 separate tables for each of the 30 chronic conditions and includes the ICD-10-CM codes associated with the chronic condition.
