NRS-20837 | X-Ray report cards [Callan Park Hospital]

This series contains x-ray report cards for patients and officers at Callan Park Hospital.The folded cards are titled on the front 'X-Ray Report Folder' and contain the printed fields: Surname (Block Letters), Christian Names, Patient's No. [number] , X-ray No. [number], Date, Age, Date of Birth, Sex - M/F [male/female], and Ward. The front cover when lifted contains an attached sheet with doctors' notes relisting all the patient fields again along with: Clinical Notes, X-Ray Request, M.O [medical officer], Date, Date of X-Ray, Report including Doctor's name. The Patient's Number is relisted inside the card as 'Unit No.'. The field entries are handwritten or typed. In the case of officers' x-ray cards the Patient No. and Ward fields are stamped or typed 'STAFF'. Some staff cards were also marked by hand as 'TB DIV' [Tuberculosis Division].The card are arranged alphabetically by first letter only of the patients' last names. There are two runs of cards one run arranged: 1. Aa-Ai to 150. X-Y-Z and the other A-Z.
