Currently, immunotherapy represents one of the most promising strategies in the treatment of cancer. Specifically, the greatest success in this field has been achieved through monoclonal antibodies capable of blocking the action of molecules involved in stopping the adaptive immune response, the so-called "immune checkpoint inhibitors" (ICIs). Despite their proven efficacy, they are not free of adverse effects. Acute kidney injury (AKI) represents one of the most critical effects in the evolution and prognosis of the disease. In this context, the etiological diagnosis of the damage is crucial for the management of the patient with AKI. However, at present, diagnosis based on laboratory tests is insufficient and biopsy, with its associated risks, represents the only alternative to identify the type of damage. This project proposes the development of a rapid, non-invasive, and specific diagnostic tool for renal damage associated with treatment with ICIs, using a panel of urinary biomarkers that will be validated in a multicenter study in order to: 1) identify the type of damage (biomarkers for differential diagnosis), 2) detect subclinical damage (biomarkers of subclinical damage) and 3) detect patients at risk associated with treatment with ICIs (biomarkers of risk). In short, this study proposes to address a problem that needs to be resolved in the context of the diagnosis of kidney damage and subsequent management of the patient treated with ICIs. For this, it has a conceptually innovative proposal in which the applicant team is an expert and with professionals from the Oncology and Nephrology Services of 9 Hospitals.
