VPRS 18381 Admission Index Cards Alcohol and Drug Services

This series comprises of index cards for patients attending Gresswell Drug and Alcohol Rehabilitation Centre (VA 5077) , St Helens/Pleasant View (VA 2848) and the Smith Street Clinic (VA 5092). This series includes patients receiving treatment at an outpatient facility, as well as those who have been admitted as inpatients. It is often the case that a person presents as an outpatient, but has also spent periods as an inpatient.The index cards include name, date of birth, date of admission, previous admissions, type of admission, type of institution, name of institution, date of discharge, date of transfer, details of trial leave, details when boarded out, dates of escape and return, date and cause of death, diagnosis, and report of superintendent as to physical and mental condition of patient.Other information may include patient's address on admission, permanent address, age on admission, religion, marital status, birthplace, year of arrival in Australia, whether naturalized; last school attended, education standard achieved; professional or vocational qualifications, last occupation, whether still employed; next of kin and their address.
