Negotiated_Type: negotiated, derived, fee schedule, percentage, or per diem
Negotiated_Rate: Dollar or percentage based on the Negotiation_Type
Expiration_Date: Date agreement for the Negotiated_Rate ends
Provider_NPI: The provider's NPI
TIN_Value: The TIN value of the provider's business
TIN_Type: NPI or EIN
Provider_Taxonomy: The primary taxonomy of the provider
Provider_Business_Practice_Location_Address_State_Name: 2-digit state code
Provider_Business_Practice_Location_Address_Postal_Code: 5-digit postal code
