The Cost of Not Learning: Rare Failures from Mariana to Brumadinho

This deposit contains the open data and materials underlying the article "The Cost of Not Learning: Organizational Learning from Rare Failures and the Mariana-Brumadinho Sequence," a documentary single-case study of the 2015 Mariana and 2019 Brumadinho tailings dam disasters involving Vale S.A., analyzed through the lens of organizational learning. The study uses no proprietary or personal data. Every source analyzed is a public document: independent expert-panel and committee reports, parliamentary inquiry reports (including the Minas Gerais Legislative Assembly inquiry), securities filings, judicial decisions, reparation agreements, and regulatory enforcement records. This deposit consolidates those sources and the datasets derived from them so that the analysis can be traced and reproduced. The main file is a workbook (RAC_OpenData.xlsx) with six sheets: (1) a README describing purpose, provenance, and conventions; (2) a source register listing all documentary sources with type, origin, date, public access link, and use in the analysis; (3) a dated chronology of the Mariana-Brumadinho sequence, 2015 to 2026; (4) a cost reconciliation of prevention and realized failure costs, shown in nominal reais and in constant end-2025 reais, with the IPCA deflation factors and formulas exposed for audit; (5) a coding scheme mapping each source to the theoretical mechanism it informs; and (6) the underlying values plotted in the article figures. The deposit also includes the article figures in high resolution. Derived-data note: monetary values are recorded as disclosed (nominal, with year) and, where applicable, deflated to constant end-2025 reais using annual IPCA indices (IBGE). The upstream-dam decharacterization programme, priced in US dollars, is converted at R$ 5.00 per US dollar and is a forward, ex post figure that is not deflated; the one-day market-value loss is likewise not deflated. National dam-stock figures are from the Agência Nacional de Mineração (ANM, 2023).
