Hypertension is defined as either reporting use of antihypertensive medication or having a systolic blood pressure ≥160 mm Hg or a diastolic blood pressure ≥95 mm Hg; diabetes is defined as either reporting use of antidiabetic medication or having a serum glucose level ≥11.0 mmol/l.
