To explore markers and potential mechanisms of antipsychotic efficacy in patients with schizophrenia, we conducted a trans-omics analysis integrating genotyping, proteomics, and metabolomics in 208 patients treated with oral paliperidone for 6 weeks. The results showed that higher levels of phosphatidylcholine and sphingomyelin were associated with better antipsychotic efficacy. These associations were further genetically validated in two independent cohorts.
