Core suite of integration indicators

This release by Public Health Scotland (PHS) presents annual rates for the Core Suite of Integration Indicators for each Integration Authority area and Scotland. These indicators were developed to help Integration Authorities to review progress towards achieving each of the National Health and Wellbeing Outcomes which focus on improving how services are provided and the difference that integrated health and social care services should make for people. Some indicators are based on survey feedback while others are derived from data routinely collected for other purposes, such as hospital activity data and National Records of Scotland death records.
