Primary Care Systems for Person-Centred Provider Practices
收藏资源简介:
Person-centred care aims to shift the conventional medically dominated, disease-oriented, fragmented care towards one that is collaborative and relationship-focused. However, there is limited understanding of how person-centred care is practiced and supported in Malaysian public primary care clinics. This pragmatic, multi-centre evaluation of person-centred care highlighted the need for a co-creation approach involving collaboration between clinic managers, healthcare providers and service users to tailor an intervention to suit their specific needs and circumstances. The tools developed in the study also helped the individuals to reflect on their person-centred practices and develop a greater sense of personal responsibility, engagement, and motivation.
