Intervention group:1. Oncologist give a standardized handover summary to primary healthcare physicians when patient discharge, which includes basic information on the treatment, patient functional status, underlying problems, and care plan that transferred to be managed in primary healthcare centers (PHC).
2. Health education (regular healthcare, symptom recognizing and management, primary healthcare follow-up services etc.)
3. Symptom alert;Control group:Usual care
Primary outcome(s): Health-related quality of life
