Dataset description. This item is a de‑identified Excel workbook (.xlsx) containing the full survey dataset underpinning the manuscript on mental health professionals’ (MHPs’) work with service users’ (SUs’) identity. Data were collected via a Qualtrics (XM, 2024) online questionnaire over a two‑month period in 2024 from UK‑based MHPs meeting the study’s inclusion criteria, with ethical approval from the UCL Research Ethics Committee (ID: 27023/001) and informed consent obtained prior to participation. The Excel workbook holds row‑level responses (one row per participant) for demographic and job characteristics, multiple‑choice items, 5‑point Likert‑type agreement/frequency items, and open‑ended (free‑text) questions. Quantitative responses are stored as numeric codes with human‑readable labels in the column headers; qualitative responses are preserved as plain text. Qualitative data are also in the Excel workbook, unedited. Direct identifiers (e.g., names, email addresses, IPs, timestamps) have been removed, and low‑frequency categories have been reviewed to reduce re‑identification risk in line with the study’s reporting approach.Creation and curation.Raw responses were exported from Qualtrics to Microsoft Excel (Microsoft 365) and lightly curated to standardise variable names, value coding (e.g., 1–5 for Likert options), and missing values (blank/NA where applicable). Qualitative data are also in the Excel workbook. Data are supplied exactly as analysed for the manuscript’s descriptive statistics and thematic summaries; any derived totals in the paper can be reproduced by filtering or pivoting within Excel. Users wishing to reuse the data can filter by role, setting, and client group, and can conduct secondary quantitative summaries or qualitative re‑analysis of the open‑text fields.
