Mitochondrial dysfunction, an early pathological event in TAR DNA-binding protein 43 (TDP-43, also known as TARDBP) proteinopathy, drives protein aggregation and neurodegeneration. Pathological accumulation of TDP-43 within mitochondria constitutes a primary mechanism driving pathology by causing mitochondrial dysfunction. While mitophagy normally maintains mitochondrial homeostasis by selectively eliminating the damaged mitochondria, a key question remained: why is mitochondrial TDP-43 not efficiently cleared along with damaged mitochondria?
