This dataset contains anonymized administrative claims data used to analyze adherence to statin therapy in the Czech Republic between January 1, 2017, and December 31, 2020. The data were provided by six employee health insurance funds, covering approximately 40% of the country’s insured population. The study includes two cohorts: incident statin users (those with no statin use in the previous 12 months) and prevalent users (those with ongoing use).Adherence was measured using the Proportion of Days Covered (PDC), a standard metric based on prescription refill records, calculated either for each medication sequence or over the entire observation period. The dataset captures over 890,000 prevalent and nearly 288,000 incident statin users, and includes key variables such as age group, sex, statin type and dose, treatment intensity, and medication sequence.Patients were grouped by age (18–39, 40–59, 60–79, 80+), and treatment intensity was classified based on statin dosage (e.g., atorvastatin ≥40 mg or rosuvastatin ≥20 mg as high intensity). The data allow assessment of adherence across these subgroups and evaluation of predictors of adherence using logistic regression.The primary objective of the study was to describe patterns of adherence to statins and identify factors that influence it—namely age, sex, treatment intensity, and treatment sequence. Key findings show that adherence improves with age and with later treatment sequences, while females and those on high-intensity statins are slightly less likely to maintain high adherence.This dataset supports public health and clinical research aimed at improving adherence to lipid-lowering therapy and can serve as a resource for further analysis, benchmarking, or model validation. All data are fully anonymized and comply with applicable ethical and legal standards.
