A case report of Spontaneous Rupture of Liver Cancer During Pregnancy Complicated by Hemorrhagic Shock

A case of hemorrhagic shock caused by spontaneous rupture of hepatocellular carcinoma in late pregnancy complicated with hepatitis B virus infection was reported. The pregnant woman was 31 years old and had a history of hepatitis B for more than 20 years. She was admitted to hospital because of "sudden epigastric pain for more than 2 hours at 32+2 weeks of pregnancy". Her blood pressure was 80/40mmHg.The diagnosis was abdominal hemorrhage, hemorrhagic shock, fetal distress and pregnancy complicated with hepatitis B. During the operation, it was found that the tumor of left lobe of liver was ruptured with active hemorrhage, and the rupture was 2*2cm.Emergency cesarean section assisted delivery of a premature live baby boy (discharged after 19 days of treatment),and then partial hepatectomy was performed. Pathology confirmed nodular hepatocellular carcinoma. The parturient was discharged 15 days after operation. Antiviral and targeted therapy were given simultaneously after operation. No tumor recurrence and complications were observed during follow-up for 12 months. This case suggested that pregnancy complicated with hepatitis B should be alert to the risk of liver cancer during pregnancy. Multidisciplinary cooperation can obtain good maternal and infant outcomes.
