Questionnaire- LBW cohort study

This is the questionnaire used in the LBW cohort study. It contains questions on baseline information, obstetric history and details of delivery of the index child, antenatal history, natal history, medical history of the mother, baseline information of the participant child, and feeding practices and dietary habits. The anthropometric measurements, biochemical investigations, echocardiogram results, and results of the developmental assessments are also included in the questionnaire.
