The Injury & Safety Report is a mandatory post trip legal document observers fill out to report any injuries they have incurred, illnesses they have had, or safety compromising situations such as no wheel watches they have encountered on an observed trip.
《伤害与安全报告》系一项强制性的行程后法律文件，观察员需据此填写所遭受的任何伤害、所罹患的疾病，或遭遇的安全隐患情况，例如在观察行程中遇到的缺失车轮监控等情形。
